Liability Release Notice
I hereby release, forever discharge and agree to hold harmless, Simpson Street Church of Christ, its leadership, bus driver, staffs and volunteers thereof, from any and all liability, claims, or demands for personal injury, property damage and expenses, actions or rights of action which are in any way related to riding the church bus/van and attending the church event.
In case of an emergency, I understand that every effort will be made to contact a family member. However, if a family member cannot be reached, I herby give SSCOC permission to act on my behalf in seeking and administering medical treatment in the event that such treatment is deemed necessary or advisable for my health, safety and welfare. I release SSCOC from liability in acting on my behalf in this regard and rendering such medical treatment.
This Liability Release Form applies to all instances where one is a rider of the SSCOC church bus or van.
